Golden Hours; Ayushman Aarogya Mandir, Jamsar

The gram panchayat Jamsar which is located around 8 km from the Taluka- Jawhar. the village where currently I am working as a health fellow from past two years. The gram panchayat caters a population of approximately 20,000 to 28,000 as per panchayat records of 2022. The village Jamsar itself has a mixed culture of tribal communities mainly K-thakur, Warli, Konkan, and Koli. The village has a Zilla Parishad School for getting education to the students. It also has a bank and health facility for the villagers. Recently the gram panchayat come forward to build a temple for religious purposes, and water storage tank for providing drinking water to every house. The active gram panchayat committee of the village take essential steps time to time for the development and welfare of villagers. The pond cleaning, roads constructing, and restoration of ancient shiva mandir are some of the colorful portraits one can see now a days.

Introduction

This is the story of the primary health centre, currently “Ayushman Aarogya Mandir” where I am working as a JSW Foundation Fellow since 2023 situated at gram panchayat Jamsar.  “Ayushman Aarogya Mandir” Jamsar is not only providing free treatment and medication to the village community or patients. But it also focuses on the better quality of healthcare services and patient’s satisfaction. In the village Jamsar, Primary health centre/ Ayushman Aarogya Mandir caters to a population of approx. 35,000. The centre comprises a dedicated team of approx.30-35 health staffs who work hard to deliver the quality of health care. Also, it has a bed capacity of approx. 10-12 for emergency, maternal and trauma care. Under “Ayushman Aarogya Mandir” Jamsar- 08 Aarogya Vardhini Centre/ Sub-Centre also spread over 28 villages and 83 padas to deliver quality of healthcare services for tribal community. Similarly, Taluka- Jawhar has 05 other Ayushman Aarogya Mandir/PHCs namely (Sakur, Sakhersheikh, Jamsar, Nandgaon, Jawhar Nagar-parishad) situated at different Gram Panchayat to deliver the health services to the tribal community.

A day at the Ayushman Aarogya Mandir/PHC Jamsar

The working hours at PHC showed a huge rush of patients for getting consultation from Dr. Anita and Dr. Vaishnavi medical officer posted at “Ayushman Aarogya Mandir” Jamsar. The process of availing the health consultation is very simple. The patient needs to first registered themself at the registration counter through sharing his/her name, village address. Mobile number etc. the registered details and unique registered number is written on the prescription. Later the patient is waiting for his/her turn at the waiting area of PHC. The patient is called to see the doctor. After hearing all the issues medical officer prescribes medicines and test as per the requirement. All the test and medicine are available free of cost at the PHC’s Laboratory and Pharmacy.  

For NCD Clinic

For all those NCD first time visiting patients 30-years and above at PHC, their triage is conducted by the ANM/staff nurse/nursing interns posted at centre in immunization room. Here the patient’s weight, and height is recorded to calculate the body mass index. The posted nursing interns also counsel the patients for diet modification in case of over and under-weight.

Along with this, for hypertensive and diabetic screening Blood Pressure and Random Blood Sugar reading are measured and clearly written on patient’s prescription. Then the patient is asked to see the doctor and narrate his/her health issues without hiding any past suffering and treatment. The doctor give consultation to the patient and prescribe test, medicines as per the diseases.

For Immunization Clinic

The Ante-Natal Care, for pregnant mother and vaccination for infants PHC is having a dedicated maternal and child health wing associated with female wards. All the essential services and health schemes like Janani Suraksha Yojana (JSY), Pradhan Mantri Matritva Vandana Yojana (PMMVY), Pradhan Mantri Surakshit Matritva Abhiyan (PMSMA), Janani Shishu Surakhsha Karyakaram (JSSK), National Immunization Programme (NIP) etc. benefits are drawn under government health schemes. The ANM/Staff Nurse help beneficiaries to avail these schemes.

Special Clinic

The Centre also focuses on special clinic concept where Eye clinic, dental clinic, counselling session for mental health, neglected disease cases are seen with the support of specialist and also further referrals are made to tertiary care hospitals nearer Nashik. The national health programmes like- Rashiya Kishore Swasthya Karyakaram (RKSK), National Vector Borne Diseases Control Programme (NVBDCP), National Programme for Prevention of Cancer, Diabetes, Cardio vascular Disease, and Stroke (NPCDCS), etc are also focused.  

Expanded range of services

The Ayushman Aarogya Mandir visioned to deliver various expended range of services to the community. The expansion of services has been planned in incremental manner. As a first step, Screening, Prevention, Control and Management of Non-communicable Diseases and Chronic Communicable diseases like Tuberculosis and Leprosy has been introduced at Ayushman Arogya Mandir.

  • Screening and Basic management of Mental health ailment

Regular training of grassroot health staffs mainly ASHA workers and Aanganwadi Workers for malnutrition, breast cancer screening, various national health programs are conducted time to time at Ayushman Aarogya Mandir/PHC. The daily OPD consultation for the centre is more than 60+ for general clinic and 30+ for Maternal and Child Health. Referral cases are also made from Ayushman Aarogya Mandir- Jamsar to Sub-District Hospital- Jawhar, also called as P.C. Cottage Hospital- Jawhar, and Civil Hospital Nashik. At Ayushman Aarogya Mandir/PHC, Nursing students from Jawhar got posted as per their courses curriculum and understanding.

Fellow’s Experience;

As a JSW Foundation Fellow I got an opportunity to capacitate our grassroot health worker’s skills and knowledge as per health programs mainly Accredited Social Health Activist (ASHA) workers for screening of hidden cases of tuberculosis, updated information to them under National Tuberculosis Elimination Programme, its components and intervention for the community through workshop rangoli, health talks etc. I also associated my working knowledge, and learnings with various health staffs like- Multi Purpose health Workers, Community Health Officers, pharmacist, lab technician and health admin staffs of PHC to understand the working culture, reporting mechanism, monthly meetings, inventory management and online portal entries.
